Community Care Coordination technology platforms can play a valuable role in managing referrals and connections with community based organizations that can work with residents to help them thrive in the community. The system includes the ability to manage communications between community workers, residents, and personal support team individuals via texting, email, and video conferencing. Offering these supportive services helps to improve economic, health, and social outcomes leading to longer occupancy and less turnover.
