This Track focuses on ways that housing and care coordination can minimize many of the problems that are often intertwined with housing instability and homelessness. People struggling with housing insecurity often have a mix of complicating factors ranging from substance abuse and mental health issues to chronic disease to job, transportation or legal issues. By integrating some of the most promising practices and tools for “whole-person” approach into the housing strategy, many of the most common problems can be more effectively addressed.
